Responsibilities :
- Ensure QIP-NJ contractual requirements are being met.
- Track and ensure deliverables are being met per HORIZON ISC program.
- Monitor teams' performance in integrating care to patients.
- Assist in Coordinating services with outpatient service lines (physicians, behavioral health
- Coordinates treatment plan adherence.
- Monitors internal data dashboard to track trends and determine best practices to enhance
delivery of patient services.
- Identifies community resources to address patients' social determinants of health.
- Maintains program database of community partners and referrals.
- Maintains documentation of all patient encounters and completes reporting requirements
according to organizational and contractual requirements.
Ensures safe and professional patient care by enforcing and monitoring compliance to
professional standards, identifying clinical and educational needs of staff; recommends
methods for improving staff competence and patient care and assists in developing,
implementing and evaluating programs.
- Maintains strict confidentiality in accordance with state and agency policies.
- Attends and represents the organization at professional conferences, in-service training,
and meetings at the request of or with the approval of supervisor.
- Flexibility with covering weekend / evening shifts.
- Managing On-Call responsibilities.
- Other duties as assigned.
Position Qualifications :
- Minimum of 3 years of supervisory or lead experience.
- Three years of healthcare management experience or program management
- Excellent communication and interpersonal skills.
- Experience working with behavioral health and / or substance use population.
- Experience working in a medical or social services environment preferred.
- Strong attention to detail.
- Proficient with computer databases.
- Analytical problem-solving skills.
- Ability to manage time, prioritize, and meet deadlines.
- Ability to work independently and as a team member.
- Good attitude, professionalism, promote team cohesiveness.
- Proficiency in Spanish preferred.
Education :
- Bachelor's degree in behavioral health field / healthcare management required.
- Master's Degree preferred.
