Primary Care Nurse - Optum

Responsibilities

• Perform comprehensive assessments of patients' health status and develop personalized care plans
• Administer medications and treatments as prescribed by physicians
• Monitor and document patients' vital signs, progress, and response to treatment
• Collaborate with multidisciplinary teams to ensure coordinated and efficient care
• Educate patients and their families on disease management, prevention, and healthy lifestyle choices
• Coordinate referrals to specialty care and follow-up with patients as needed
• Maintain accurate and up-to-date medical records and documentation

Requirements

• Bachelor's degree in nursing from an accredited institution
• Current and unrestricted registered nurse (RN) license
• Minimum of 2 years of experience working as a primary care nurse in a healthcare setting
• Strong clinical and critical thinking skills
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced and dynamic environment
• Strong attention to detail and organizational skills
• Knowledge of electronic health record systems and medical terminology
