We are seeking a knowledgeable and compassionate Geriatric Pharmacist to join our team. As a Geriatric Pharmacist, you will play a vital role in providing pharmaceutical care to older adults, ensuring the safe and effective use of medications. You will work closely with healthcare providers, patients, and their families to optimize medication therapy and improve patient outcomes. The ideal candidate will have a strong background in geriatric pharmacology and possess exceptional communication and organizational skills.

Responsibilities

  • Conduct comprehensive medication reviews for geriatric patients
  • Assess medication regimens to identify and resolve any drug therapy problems
  • Collaborate with healthcare providers to develop personalized medication plans for older adults
  • Educate patients and caregivers on medication use, side effects, and precautions
  • Monitor medication adherence and recommend interventions as needed
  • Participate in interprofessional care teams to provide holistic care to geriatric patients
  • Stay up to date on the latest research and guidelines in geriatric pharmacology

Requirements

  • Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy program
  • Active state pharmacy license, in good standing
  • Board certification in geriatric pharmacy (CGP) strongly preferred
  • Minimum of [X] years of experience as a pharmacist, [X] of which must be in geriatric pharmacy
  • Strong knowledge of geriatric pharmacotherapy, including common age-related changes in drug metabolism and potential drug interactions
