Clinical Practitioner - Optum

Responsibilities

• Conduct comprehensive assessments of patients' health conditions and develop individualized care plans
• Administer medications, treatments, and therapies as prescribed by physicians or other healthcare providers
• Monitor patients' progress and modify treatment plans as necessary
• Collaborate with interdisciplinary teams to ensure the delivery of high-quality and coordinated care
• Educate patients and their families on disease management and self-care techniques
• Maintain accurate and up-to-date medical records
• Adhere to all applicable legal, ethical, and professional standards

Requirements

• Minimum of a Bachelor's degree in Nursing or a related field
• Valid state licensure as a Registered Nurse (RN)
• Proven experience as a Clinical Practitioner or similar role
• Strong knowledge of medical terminology, procedures, and protocols
• Excellent communication and interpersonal skills
• Ability to work effectively in a fast-paced and demanding environment
• Strong attention to detail and organizational abilities
• Demonstrated commitment to providing patient-centered care
